ACTION AND MECHANISM

Nasal decongestant, association of direct acting sympathomimetic amine, imidazole derivative (oxymetazoline) and an antihistamine H1 (chlorpheniramine). The oxymetazoline applied on the nasal mucosa stimulates the alpha-adrenergic receptors causing the constriction of the vessels that irrigate the mucosa. Reduced blood flow decreases congestion and facilitates nasal ventilation.
Chlorphenamine prevents vasodilatation induced by histamine and suppresses capillary permeability, with reduced edema.

PHARMACOKINETICS

Nasal route: Both components can be absorbed from the nasal mucosa and the gastrointestinal tract to the systemic circulation, with the possibility of systemic effects, especially with the use of excessive doses administered during prolonged treatments (several weeks). Older people and children are more sensitive. The beginning of the action is 5 minutes and the duration of the action is 7-12 hours. Chlorphenamine is distributed in the cerebrospinal fluid, causing drowsiness. The elimination half-life is 5-8 h (oxymetazoline) 20 h (chlorpheniramine).

POSOLOGY

- Adults, adolescents and children> 6 years: one application in each nasal orifice, maximum 2 times a day (every 12 h).

- Children <6 years: use not recommended.

The treatment should not exceed 3 days, unless directed by the doctor.

 

RULES FOR THE CORRECT ADMINISTRATION

Nasal route

If it is the first time it has been used or has not been used for a long time, it is necessary to charge the sprayer: keeping the container away from the body, press the sprayer several times until a finely powdered liquid comes out.

Before its application, the existing nasal fluids should be eliminated, the nose blowing well.

After each use clean the end of the applicator with a clean, damp cloth before closing the container.

 

CONTRAINDICATIONS

- Hypersensitivity to oxymetazoline or to other adrenergic dedesdants, as well as to chlorphenamine.
- It should not be applied in dry inflammatory processes of the nasal mucosa (dry rhinitis).
- Children under 6 years old.

PATIENT TIPS


- To reduce the risk of rebound congestion (increased nasal congestion) should not be administered for more than 5 days in a row.
- Clean the nose before application.
- Do not share the container with another patient, to avoid possible infection infection.
- Wash the applicator with hot water and clean after each application.
- If insomnia appears, you should not use the medication late in the afternoon or at night. After excessive use weakness, dizziness, nervousness and insomnia have been observed.


SPECIAL WARNINGS


- Clinical monitoring is advised in patients with severe cardiovascular disorders, diabetes mellitus, hyperthyroidism and prostatic hypertrophy. The systemic absorption of the drug can aggravate the disease.
- Use it only in acute cases. No more than 5 consecutive days.
- Continued use can cause rebound congestion, itching, irritation and nasal dryness. This motivates the patient to increase the frequency of use and the dose with risk of toxicity, rebound congestion and tolerance.
- Some patients may experience a slight itching sensation after application, which often disappears after several applications.

INTERACTIONS

If sufficient systemic absorption occurs, it can interact with the following drugs: - Tricyclic antidepressants and MAOIs, beta-blockers: possible potentiation of alpha sympathomimetic effects with risk of arrhythmia, tachycardia, severe hypertension and hyperpyrexia.

PREGNANCY

There are no adequate and well-controlled studies in humans. It is not known if topical oxymetazoline can cause fetal harm when administered to pregnant women; the possible systemic absorption must be taken into account. In a study in which single doses of oxymetazoline were given to 12 pregnant women during the 3rd trimester, there were no reported maternal blood pressure and pulse rates, fetal aortic blood flow velocity or fetal heart rate; the researchers concluded that the use of oxymetazoline, at the recommended frequency, does not pose a significant risk. Although fetal harm is not expected with the use of moderate doses for a short time (no more than 3-5 days), the use of this medication is only accepted in the absence of safer therapeutic alternatives.

LACTATION

It is not known if oxymetazoline is excreted in breast milk. The possible systemic absorption of nasal oxymetazoline should be taken into account since young children are more sensitive to the pharmacological and toxic effects of sympathomimetics. Precautionary use, always using moderate doses for no more than 3-5 days.

CHILDREN

Children, especially the youngest ones, tend to be more prone to the systemic absorption of vasoconstrictive preparations administered nasally, with the side effects or adverse effects that result from it. The use of 0.05% oxymetazoline nasal solutions is not recommended in children under 6 years of age, unless they are subject to strict clinical control. The use of 0.025% oxymetazoline nasal solutions is not recommended in children under 2 years of age. It is recommended to always use moderate doses for no more than 3-5 days.

ELDERLY

The safety and efficacy of nasal oxymetazoline use in the elderly have not been specifically established, however, the elderly may be more sensitive to the adverse effects of sympathomimetics. Precautionary use, always using moderate doses for no more than 3-5 days.

ADVERSE REACTIONS

The side effects of this medication are, in general, mild and transient. The alterations reported most frequently with the use of adrenergic decongestants are:
- Otolaryngology: Occasionally, burning sensation, pain, sneezing and dryness of the nasal mucosa. With continued use, rhinitis medicamentosa (rebound congestion), which can lead to atrophic rhinitis or sinusitis.
For systemic absorption or overdose:
- Cardiovascular: palpitations, arrhythmias, transient hypertension.
- Central nervous system: headache, insomnia, drowsiness, dizziness, weakness, feeling of fear (exceptional with therapeutic doses).
- Gastrointestinal: exceptionally, nausea, vomiting.
