ACTION AND MECHANISM

  • Analgesic: Capsaicin applied topically triggers local irritation, manifested as erythema and a burning sensation, sometimes with itching. It is a selective agonist of the vanilloid receptor 1 (TRPV1) transient receptor potential channels. Initially, it activates cutaneous nociceptors expressing TRPV1, producing pain and erythema caused by the release of vasoactive neuropeptides. After exposure, cutaneous nociceptors lose sensitivity to various stimuli, a phenomenon known as desensitization, which relieves pain. Capsaicin-induced nociceptor alterations are reversible and normal function is recovered within weeks.

PHARMACOKINETICS

  • Release: In vitro, approximately 35% of the capsaicin content has been shown to be released during an application of up to 8 hours.
  • Bioavailability: Between 27% and 34% when applied topically.
  • Absorption: Absorbed percutaneously at a rate of 7-11 mcg/cm²/h.
  • Metabolism: Mainly in the liver.
  • Elimination: It is excreted in the form of metabolites in urine and feces.

INDICATIONS

  • Symptomatic relief of localized myalgia and musculoskeletal pain.

POSOLOGY

Cream:

  • Adults: 3-4 applications/day.

RULES FOR CORRECT ADMINISTRATION

  • Apply to the affected area and spread with a light massage to promote the penetration of the drug.
  • Wash your hands after each application.

ADVICE TO THE PATIENT

  • Avoid contact with eyes and mucous membranes.
  • Use only on intact skin, not on open wounds, mucous membranes or eczematous skin.
  • Do not apply simultaneously with other topical preparations in the same area.
  • Do not use for prolonged periods or in large areas.
  • Wash hands after each application.
  • Do not use during pregnancy or lactation without consulting your doctor.
  • Avoid scratching the application area to prevent injuries.
  • Do not apply other heat sources during treatment.
  • Consult your doctor if the pain is accompanied by redness, swelling, heat in the joints, severe back pain that radiates to the legs, numbness, or a tingling sensation.

CONTRAINDICATIONS

  • Hypersensitivity to capsaicin, fruits of the genus Capsicum or any component of the medication.

SPECIAL WARNINGS

  • If symptoms persist more than 7 days or irritation or worsening occurs, evaluate the patient's clinical situation.

INTERACTIONS

  • It is not recommended to use together with other topical products applied to the same area.

PREGNANCY

  • Safety in animals: Prenatal treatment of rats with high subcutaneous doses showed functional neuronal defects, delay in body growth and sexual maturation.
  • Safety in humans: Capsaicin crosses the placenta and may cause toxic effects on the peripheral nerves of fetuses. Its use is not recommended during pregnancy unless there are no safer alternatives.

LACTATION

  • Safety in humans: Capsaicin is excreted in breast milk. The risk to infants cannot be excluded. Decide whether to stop breastfeeding or stop treatment.

CHILDREN

  • its use is not recommended.

ADVANCED AGE

  • No specific problems have been described in the elderly that require dosage readjustment.

ADVERSE REACTIONS

  • Skin irritation: Sensation of stinging and erythema in the application area (50% of cases), usually decreases over time.
  • Others: Dry skin, sneezing, tearing or coughing due to inhalation of dried cream residue.
  • Allergic reactions: Hives or blisters (0.01%-0.1%).

ADVERSE REACTIONS RELATING TO EXCIPIENTS

  • Methyl parahydroxybenzoate: May cause allergy to parabens.
  • Propylene glycol: May cause skin irritation.
  • Propyl parahydroxybenzoate: May cause allergy to parabens.

OVERDOSE

In case of topical overdose, local adverse effects may be more intense, but no significant systemic toxicity is expected. If accidental ingestion occurs, immediate medical attention should be sought.
